Acton PCN, part of North West London ICB, enlisted Suvera to support people living with type 2 diabetes across their entire PCN footprint. The service reduced diabetes distress scores (DDS), body weight and saw high patient satisfaction.

Delivering a virtual group model

Serving an overall list size of 80,000 patients, Acton PCN encompasses 14 GP practices today. The team asked Suvera to launch a virtual group consultation service that could support proactive diabetes management, and enable early intervention and prevention across their footprint.

Suvera’s innovative virtual clinic provided diabetes education at scale. Individuals were invited to discuss goals and develop plans in 90-minute sessions. Group clinics also included review of a diabetes results board for recent blood tests, blood pressure, weight/BMI and medication. Clinics were provided in addition to patients’ individual annual diabetes reviews

Improving outcomes

The virtual group clinic surpassed ICB Enhanced Services and delivered:

  • 7.4 reduction in average DDS (based on the average DDS of 41 patients who submitted a score for two or more sessions)
  • 6.5% reduction in average weight loss (based on average weight of 27 patients who submitted a recent weight for two or more sessions)
  • 90% patient satisfaction rate

And due to the success of the partnership, Suvera has been invited to cover non-diabetic hyperglycaemia and Level 1 Diabetes Enhanced Services.
