Your journey with Suvera

Your invitation

When your practice begins to work with us, they’ll send you a text message or a letter explaining our service in detail and how we’ll be helping you manage your long-term condition from home.

Patient portal

In the text message you receive, they’ll be a link to your practice’s patient portal. You can submit data, like blood pressure readings or fill out your condition specific questionnaires, through this portal.

Our patient portal is safe and secure. Anything you submit will stay confidential and only be shared with the care team looking after you.

Your annual reviews

When it’s time for your appointments, these will be done over the phone or by video call with a member of our highly trained team.

They’ll review all the data you’ve submitted and make sure you’re on track and feeling healthy.

How to use our patient portal

We've made our platform as easy as possible to use.

Logging in

When you’re logging into the patient portal, all you need to do is enter your date of birth and full name to get started. You don't need to create an account or have a password to begin. Once you’ve signed in, you’ll be taken straight to the homepage.

Using the homepage

On the homepage, you’ll see your to-do list. This lists out all the important information we need to check before an appointment. This shouldn’t take you too long to complete.

Just click on the blue start button to begin.

Recording your blood pressure

High blood pressure isn’t usually something that you feel or notice, so it’s important to keep monitoring it. You should always take and submit three readings at once. To get the most accurate readings, you should leave around one minute between each blood pressure reading.  


Filling in your health questionnaires

All our questionnaires have been designed to complete as quickly and easily as possible. If you don't think you have enough time, we'd suggest coming back later because you can't save your answers. 


To skip a question, just press next.
